ACCOUNTS & BILLING:

Please be aware that payment is due the same day as you scheduled appointment.

We require a valid credit card on each patient’s account to hold your appointment time and maintain our cancellation policy. We will only charge your credit card for missed appointments, late cancelled appointments, or late payments. We sincerely thank you for helping us make sure that our healthcare practitioner’s time and energy are respected.

CANCELLATION POLICY:

Longevity Physical Therapy has a high commitment to client care and willingness to commit 1.5-2 hours in their schedule to your case for every appointment. This includes a full one hour appointment that we spend with you, 30 minutes in the two days leading up to your appointment to review your case and ensure important details in your case are at the front of your physiotherapists mind, and an additional 30 minutes after your appointment to put your program, your videos and any other resources that your physiotherapists suspects will be helpful for you. We put your program and extra resources together for you immediately after your appointment to ensure that you have it as soon as possible and limit any delay between your appointment time and when you can begin your at-home care. 

As such, each appointment time is extremely valuable and includes 90-120 minutes in your physiotherapist’s schedule in the day of and days leading up to your scheduled appointment. Your appointment time is reserved just for you.

A late cancellation often occurs both after your physiotherapist has already dedicated time to working on your case and often does not offer us enough time to successfully fill the appointment time that we reserved for you with another client. Attempting to fill the time slot that we reserved for you also often results in extra time and work after hours by your physiotherapist to try to fill the time slot that was reserved for you on short notice. 

As such, we require a minimum of 48 hours notice for any cancellations or changes to your appointment. Patients who provide less than 48 hours notice or miss their appointment will be charged for their appointment in the full amount. We offer courtesy appointment reminders 7 days prior to your appointment via text, email, or both (your choice) to ensure that you are reminded of your appointment five days before the cancellation policy period giving you ample time to cancel or reschedule your appointment if needed.

We require all clients to honour the cancellation policy and greatly appreciate each client doing so as it enables us to continue to offer full one hour appointments along with the dedicating additional time before/after your appointment to your case so that we can offer you the highest level of care possible. 

You can cancel your appointment with just a few clicks by logging into the online booking and cancelling it there or you can email us at info@longevitysaskatoon.com (please note that clinic hours are 6am-5pm Monday to Friday and email is not monitored outside of these hours). For all late cancellations within the cancellation policy period, please email us.

CURRENT COVID-19 POLICIES

As a healthcare facility that requires close contact with patients for long durations we will continue to practice many safety protocols. We kindly request that all clients continue to follow the protocols below. Our priority remains the health and safety of staff, clients, and the community. We are grateful for your ongoing diligence!

Pre-Appointment Screening:
Each client will be required to complete a very quick screening questionnaire prior to every in-person appointment. This two question questionnaire will be emailed to you within 8hrs of your appointment and must be completed prior to your scheduled appointment.

Upon Arrival:
Please wait in your vehicle. This is not a COVID policy and rather is standard practice for us as we highly value offering private, uninterrupted sessions to each client where we can remain fully focused on you. The front entrance will remain locked while we are with clients and between appointments while we are doing our duties including privately dictating chart notes, filming exercise videos for clients, cleaning, sanitizing, and getting things ready for you! You therapist will come to the front entrance and wave you in when they are ready for you and will lock the door behind you to ensure a private and uninterrupted session! Alternatively, they will call you on your cellphone if they do not see you.

Masks:
You are welcome to wear a face mask, however it is not required.
 
Sanitization:
Hand sanitizer will be available at the door upon arrival and departure. Additional measures have been put in place to ensure thorough cleaning and sanitization occurs between clients.

Communication Policy:

Connecting and communicating with our clients is a high priority to us! While we cannot be reached via text or phone, we can be reached via email at info@longevitysaskatoon.com from 6am-5pm on Monday-Friday (excluding holidays). We can also be reached via the Contact a Physio page on our website. In either instance your physiotherapist will respond by reaching out to you directly as soon as possible - typically within 24 business hours or sooner.

We ask that you do not contact us via social media to cancel or reschedule appointments as we do not monitor our social media inboxes regularly. Instead we ask that you use the online booking to cancel/reschedule/book appointments. If you need to contact us or need any assistance please email us or use the Contact A Physio form and we will be happy to assist you! Please note that we do not recommend asking questions related to your case or sharing personal information over social media as these platforms are not secure and privacy and confidentiality can become compromised.

While our online booking system will send you text reminders for your appointments if you choose, we do not receive any text replies to these SMS appointment reminders as these texts are computer generated from our online booking system.
